The Indigenous Australians’ Health Programme (IAHP) aims to increase access to culturally appropriate and self-determined health care by prioritising Aboriginal Community Controlled Health Services (ACCHSs) to lead the delivery of programs and services. This grant opportunity’s purpose is to provide the National Aboriginal Community Controlled Health Organisation (NACCHO) with funding to deliver the Community Controlled Transition Care (CCTC) program. The CCTC program will involve the co-design and delivery of a community controlled transition care model for First Nations peoples exiting custody. It will seek to improve health care access and outcomes for First Nations people in the custodial system, providing First Nations peoples with improved discharge planning, reintegration support and culturally safe health care post-release. The Australian Government has announced a total of $14,497,810 over 3 years for the CCTC grant opportunity. The grant opportunity will run from November 2026 to June 2029.
